This Targeting Safety activity provides pharmacists and pharmacy technicians with a comprehensive overview of the latest medication safety recommendations for hospitals from the Institute for Safe Medication Practices (ISMP). The session explores three newly introduced best practices—focused on preventing errors with tranexamic acid, improving medication reconciliation during transitions of care, and enhancing vaccine safety in inpatient and outpatient settings. Participants also review updates to existing practices and learn strategies for implementation, including leveraging technology, addressing barriers, and utilizing national safety resources. This CE activity empowers pharmacists and pharmacy techinicians to proactively reduce medication errors and foster a culture of patient safety within their institutions.

Credit requirements must be completed before the expiration date of the activity. Participants must watch the video in its entirety and completion of a post-test with a score of 80% or higher.
